Kiosk entry is a question by question on screen method for gathering information from the patient. It replaces the patient marking the Health Questionnaire form. Each question must be answered before the next question is asked or becomes visible. The current question is highlighted with a different background color. Option buttons are used when only one answer is allowed. Check boxes are used when a question can have multiple answers. |
The large, difficult to use arrays for complaints have been replaced with body diagrams to select complaint locations. The columns of the array (symptoms, intensity, characteristic, frequency, and status) have become separate questions about the complaint. These questions make describing the complaint self explanatory. There are separate body diagrams for the selection of each region's complaints (spine, upper extremities, lower extremities, and headaches). A summary of complaints is presented for patient review and modification. |
A text entry window will appear when the patient clicks on an answer, such as "Other", that requires additional information. The text entry window will contain instructions for what is required and will provide an example of an answer. A "Will Explain to Doctor" check box is present on the text entry window for questions the patient has difficulty answering. |
After the patient finishes answering all the kiosk questions their responses will be automatically marked on an HQ form image by the OnScreen paperless entry function. The form images can be viewed in the OnScreen program and then scanned into DocumentPlus for document generation. |
The kiosk entry program is designed to run on any work station in your office. The active kiosk entry patient is controlled from any workstations running DocumentPlus. Typically, the kiosk entry workstation is placed in the waiting area and is controlled by the DocumentPlus workstation at the front desk. When a patient is first entered into DocumentPlus, an HQ kiosk entry with status new is created. The kiosk status for a patient can be activated either on Information 2 of the patient data bases function or using utility function Kiosk Workstation Control. |
The patient is prevented from answering questions or selecting answers that do not pertain to their previous response to: male/female, family members, and occupational status. This prevents validation errors when the kiosk data is converted to a data entry form image and scanned. |
